I have extensive experience in the Aged and Disability sectors and have managed and helped set up Supported Disability Accommodation houses for clients to be able to live in the community and access various programmes with support of workers as independently as possible. I am very client focussed in all that I do and working closely with families to achieve goals and best outcomes for their loved ones.
My role as a Support Coordinator for ResiCare SA is to support clients and families in understanding their NDIS plans and how to navigate them. To be able to engage with various external agencies and providers to achieve the best outcomes. I can assist families and clients with any issues that may arise and how to overcome them.
As their plans come close to review I will follow up with all providers to get progress reports to be able to show how all goals are being achieved and to assist with gaining any additional funding and supports needed. As a Disability Accommodation manager, ensure all houses are compliant to SDA standard for clients and that they match the criteria. We also provide in home support for clients with NDIS funding within their own home with a variety of services. We continue to identify the needs of clients and families in the community and the best way when approached that we can support them to live a fulfilling lifestyle.
